Humana Plans and Coverage Levels
Humana has two insurance plans: Preferred Provider Organizations (PPOs) and Health Maintenance Organizations (HMOs).
- PPO plans: These plans allow members to see a variety of in-network and out-of-network providers without a referral. With a Humana PPO plan, in-network care has lower out-of-pocket costs.
- HMO plans: A Humana HMO plan allows members only to see a selection of in-network providers. Members must also choose a primary care physician, and out-of-network care is rarely covered.

- Medical Detox: Our clinical team provides round-the-clock medical supervision during withdrawal. Nurses monitor your vital signs, administer FDA-approved medications to ease withdrawal symptoms, and ensure your safety throughout the process. Alcohol and benzodiazepine withdrawal can cause seizures, delirium tremens, and other life-threatening complications without medical oversight. Humana PPO and HMO plans typically recognize medical detox as a necessary first step and provide coverage at in-network facilities like ours.
- Inpatient Residential Rehab: Our structured residential program immerses you in daily therapeutic programming while you live on-site at our Lake Worth campus. Each week includes multiple individual therapy sessions with a licensed counselor assigned to your case, daily group therapy on relapse prevention, emotional regulation, and communication skills, weekly family counseling to repair relationships and build support, and evidence-based therapies including CBT, EMDR, and DBT. You’ll also have access to our pool, fitness center, basketball courts, and outdoor areas. Humana evaluates inpatient coverage through ongoing utilization review. - Partial Hospitalization (PHP): When you’re ready to step down from inpatient care, PHP provides intensive day treatment five to seven days per week for four to six hours daily. You’ll continue therapy and clinical programming during the day while practicing skills in a less restrictive environment. Most Humana plans cover PHP when clinically recommended as a step-down from inpatient.
- Dual Diagnosis Treatment: Roughly half of people with substance use disorders also have a co-occurring mental health condition. At every level of care, our team screens for and treats depression, anxiety, PTSD, bipolar disorder, and ADHD alongside addiction.
